SHARS Individual Provider Enrollment for SLP/AUD Work Session



This come and go session will be a work time for referring Speech-Language Pathologists/Audiologists, working in the public schools, to complete enrollment in Texas Medicaid to become an individual provider. Any SLP/AUD who will be a referring provider (performing evaluations and recommending therapy) is currently required to complete enrollment by November 1st, 2002 in order for the school district to continue to receive payment from Texas Medicaid/SHARS. Troubleshooting and support will be provided for this process.


Important Session Information:


Participants will need the following to attempt enrollment:

  • TX license and ASHA certification information
  • Driver's license number
  • Practice location/address
  • Social Security number
  • NPI number
If you have previously been enrolled in the TX Medicaid system, you may only need to add the school-based practice location into your account.  You will need to have the existing account log-in and password information. If this can not be obtained from the original employer who enrolled you, TMHP must be contacted for assistance. TMHP 800-925-9126 (option 3) or provider.relations@tmhp.com
